Life Insurance for You and Your Family

A life insurance policy provides you and your family with peace of mind. After all, it's nice to know that your loved ones are protected and can benefit from the fruits of your labor so they can invest in their own future.

 

The Employee Life, Accidental Death & Dismemberment and Dependent Life plans are administered through Union Labor Life Insurance Company (ULLICO).

 

Shopmen's Local 508 Health & Welfare Fund provides you with the peace of mind that your family will be taken care of in the event of an untimely death by helping to meet financial obligations and maintain financial security. In addition, the Fund provides benefits for your spouse and eligible dependent children.

Employee Life: $50,000

                              $50,000 Additional Accidental Death & Dismemberment

                              $50,000 Workplace Accidential Death Benefit

                              (An additional amount equal to one times the standard AD&D amount up to a                                             maximum of  $50,000.)



Dependent Life: $25,000 for spouse and each dependent child to age 19

                            

 

In addition, each employee covered has the following unique provisions in your ULLICO policy through the Fund:

ACCELERATED LIFE INSURANCE BENEFITS

Conversion Privilege - If an individual's Life Insurance Benefit, or any portion thereof, terminates, he or she is entitled to convert all or a portion of the Amount of Insurance which has been terminated. This conversion will be an individual policy of life insurance ("Conversion Policy").  The individual will not be required to submit proof of good health to convert.

Self Pay Life Premiums for Active Employees Only - If you are absent from work due to a medical leave, layoff or strike, you may continue your Active Employee Life Insurance benefit at your own cost and expense until the earlier of 24 months; or, your seniority time period.

 

Premiums must be made prior to the first day of each month in which they are due.  Any payment made is not refundable.  Please contact the Health & Welfare Office to find out the exact cost of this premium.  It is your responsibility to ensure that payment is received in a timely manner to avoid cancellation.  All Dependent Life is excluded under this provision.
