ELIGIBILITY REQUIREMENT & WAITING PERIODS
Participation, eligibility requirements and waiting periods may vary depending on the terms and conditions outlined in the collective bargaining agreement between the Employer and Local Union. Eligible Waiting Periods for newly hired employees of the following listed Employers is outlined below:

- VISION - 1st day of the month after the completion of 30 days employment
- LIFE/AD&D - 1st day of the month after 60 days employment
- HEARING - 1st day of the month after 60 days employment
- SHORT TERM DISABILITY - 1st day of the month after 60 days employment
- DENTAL BENEFITS - 1st day of the month after six (6) months employment

Employees are eligible to enroll the first of the month following ninety (90) calendar days of employment. Upon completion of the ninety (90) days probationary period, you will be eligible as follows:
Vision - 1st day of the month after the completion of 30 days employment after probation period.
Life - 1st day of the month after the completion of 60 days employment after probation period.
Hearing - 1st day of the month after the completion of 60 days employment after probation period.
Dental - 1st day of the month after the completion of six (6) months after probation period.
