Employees are eligible to enroll the first of the month following ninety (90) calendar days of employment. Upon completion of the ninety (90) days probationary period, you will be eligible as follows:
Vision - 1st day of the month after the completion of 30 days employment after probation period.
Life - 1st day of the month after the completion of 60 days employment after probation period.
Hearing - 1st day of the month after the completion of 60 days employment after probation period.
Dental - 1st day of the month after the completion of six (6) months after probation period.
