You and your eligible dependents are eligible for dental benefits administered through Delta Dental Preferred Option (PPO) Point-of-Service Program. Dental cards are issued by Delta but you do not need the dental card to seek services, just give the dentist your Group Number, indicated below by Employer:

There are no deductibles; however there is an annual calendar maximum of $3,000 per person per Benefit Year (June 1 through May 31) on all services in Class II and III.   Dental Services under Covered Service Class I will continue to be paid at 100%, however they will be excluded from the Annual Maximum Payment. There is a $3,000 per person total lifetime on orthodontic services. In the event that you seek treatment from a dentist that does not participate in any of Delta’s Dental programs, you may be responsible for a greater percentage of the dental bill.

To use your Plan, follow these steps:

• Please read your Dental Certificate and Summary of Dental Plan Benefits carefully.
• Make an appointment with your Dentist and tell them that you have dental benefits coverage with Delta Dental.
• Supply them with your Member ID (ss#) and group number listed above.
