You and your eligible dependents are eligible for hearing benefits once every 24-month period from date of service to date of service the following year. Fidelity Security Life Insurance Company adminsters this benefit.
Hearing benefits are paid on a reimbursement basis only up to a maximum of:
$70
per 24 consecutive month period following the date of the last Hearing Exam
$750 (per ear)
per 24 consecutive month period following the date last fitted for the covered Hearing Aid
In order to receive your reimbursement, you must submit your paid original receipts attached to a claim form that is obtained from the Health & Welfare Office.
Reimbursement takes about two (2) weeks and the check will be mailed directly to you at your home address.
